Hello Jaime,
The sewer connection permit is approved for: 4540 West Arm Road. I have included some steps we follow for your
reference if it could be of any help for any upcoming connections/disconnections.
For this property in particular, we did not receive a permit to approve the disconnection, it was sent as a work order
only to shut water off(permit never applied for). The disconnect was never inspected by us. Please make sure we review
any demo permits as well as any sewer&water disconnects/connections so we can make sure our record system gets

Water Disconnection:
-Disconnect permit approved
-Locates called in/completed
-24-48 hours' notice prior to scheduling inspection
-Curb stop shut off and cut back on resident side of line
-Visual Inspection Required
-Approval needed before backfilling
Water Connection :
-Connection permit approved
-Locates called in/completed
-24-48 hours' notice prior to scheduling inspection
-Curb box must be centered over stop
-Curb stop must remain accessible and operable
-Curb box must be plumb and not buried
