The following conditions have been placed on this event:
By acceptance of this special event permit, the permit holder, on behalf of any and all organizations
and private persons, is granted authority to operate under the special event permit, and agrees to
indemnify and hold harmless the City of Spring Park from all claims arising from said event as set
forth on the application dated September 6, 2007.
Please Initial each box acknowledging your acceptance of conditions:
❑ The Orono Police Department, through its police officers, is empowered to revoke this special
event permit at any time for any safety concerns that are not immediately resolved by the permit
holder or a representative of the permit holder. The revocation of the permit shall cause the
immediate cancellation of the event originally authorized by this permit.
❑ The event sponsor will provide to Orono Police, Spring Park City Clerk and all residents living
within-00 feet of the event a written letter stating the dates and times of the event and giving a phone
number to live person (not voice mail) that c n act on behalf of Paul Thomas Homes to resolve any

Either a temporary or final certificate of occuparlcy must be issued by the Orono Building
Department prior to the start of the event. U-V,�A
❑ Paul Thomas Homes will provide shuttle service from an approved parking lot and will put on
file with the City of Spring Park written permission from the property owner prior to start of the event.
Paul Thomas Homes has proposed use of the paved parking lot north of Shoreline Drive, west of
Sunset Drive and south of Spring Street, owner Joel Buttenhoff. In addition, Paul Thomas Homes will
post signs that state something similar to "NO PARADE OF HOMES PARKING IN THIS
NEIGHBORHOOD — VIOLATORS WILL BE TAGGED AND TOWED." These signs shall be in bold
print and clearly visible to any vehicle coming into the area. The signs should also include "Parking

❑ Paul Thomas Homes will ensure all signs remain posted in�nd legible format during the
event.
❑ Paul Thomas Homes will remove all signs at the end of the event.
❑ Paul Thomas Homes will daily, during the vent, check for any trash, litter or debris caused by
the event and immediately clean it up.
❑ Paul Thomas Homes agrees to repair any damage caused by the event or by persons
attending the event to properties thereby affected.

The applicant agrees to comply with all applicable codes, ordinances, rules, and
regulations of the City of Spring Park with respect to the Event, including, but not
limited to, all codes, ordinances, rules and regulations governing alcohol, noise,
nuisance, building occupancy, public safety, and fire safety.
The undersigned represents and warrants that the undersigned is authorized to
represent, act on behalf of and bind the applicant.
The applicant states that the information contained in this application is true,
correct and complete. If any information contained in this application is found to
be false or to constitute a substantial misrepresentation of the Event, the
application will be denied and/or the permit revoked.
